Family Connections Program

The Family Service Support team provides nurturing supports to families and children engaged with ILFNFS. Support workers are contracted to empower families and children-in-care by strengthening their resilience and positive coping mechanisms. The team works collaboratively to facilitate family connection and provide opportunities for children-in-care to meet developmental goals through positive relationships. In-home supports build a safe environment by guiding household management tasks.

These tasks may include transportation or advocacy to access community resources. Engagement for children-in-care are provided with a strength based approach that includes advocacy and building competence.

Support Worker role:

  • Assist families and children-in-care to attain case plan goals
  • Provide timely documentation of contacts
  • Transport children-in-care to appointments and family visits
  • Transport families and children-in-care to access community services
  • Coordinate contact information with the Case Manager
  • Supervise or monitor family visits as outlined by the Case Manager
  • One-on-one support to families within their home
  • One-on-one support for children-in-care
